Step 1: Call nephrologist on-call or 112 if severe symptoms
Step 2: Check for shortness of breath, swelling, confusion
Step 3: Strict fluid limit if already advised
Step 4: Protect fistula arm — no BP cuff on access arm
Step 5: Document last dialysis date for hospital
